Been playing Quake Live on and off for years since beta. I've never really had any problems with performance. Now I do. I'll try to be as specific as I possibly can be.
The Problem
I get really choppy movement, it feels like there is a 50kg ball and chain tied to one of the leg of the character. It feels what it looks like when players warp. I've asked multiple players if I warp but I do not.
When I pan (move the mouse) while standing still there are no issues. While moving and looking at the edge of a wall it looks like onion layers of the edge. Almost like a ghosting effect.
I have no idea of what is causing this. All I can say that this is totally unplayable conditions.
When it occurs
Only when playing on internet servers. The issue does not appear when playing the tutorial vs Crash.
Since it only appears on internet servers. I feel that I can rule out any video settings but I'm not sure(?).
When did it start happening
Two days ago. I've tried playing a few games but it is absolutely horrible. I thought that it was an ISP or routing error meaning that I got lost packages or something like that so I waited before I'm asking on the forums. I haven't done anything to the config that I can think of. The only time I even touch settings is if something is wrong.... like now. Also, I haven't really done anything with my computer recently. Last windows update was 40 days ago.
Settings
Resolution - Desktop (2560x1440)
Vsync OFF (ON does nothing)
cl_packetdup 1
cl_maxpackets 125
rate 25000
com_maxfps 144 (usually, but I've tried 60-250 and the choppiness is 50 times worse at 60. A bit better at 250 but still horrible)
I'll post more or specific settings if you need me to, I'm just not that good remembering them while writing.

My setup
Windows 7 x64
Asus MG278 144hz Freesync
MSI 390 8GB
Intel 4790k
16 GB RAM
100/100 fiber bandwidth
Things I've checked
netgraph in game shows pretty much a flatline, not much jitter at all. If I have 40 ping on a German server it ranges from 30-50 or something like that. A Swedish server is a constant 8-12 ping.
My fps never changes. Whatever I set com_maxfps to it stays there.
Setup a ping to my own router for many hours. 0% packet loss. (of course below 1ms latency)
Setup a ping to google for many hours. 6 packets lost in total (out of many many thousands of packages). Latency average of 10ms.

In an act of desperation - Installing the latest driver for GFX. Though as mentioned, I have no problem at all vs bots so this should not be related to the graphics.
So... if anyone of you have any pointers at all. I will try it as soon as possible.
