Choose the caption bubble to begin. If you want to upload a custom bubble or alternative image then submit the caption first, then edit the comment and attach a "Place img". Once the image is approved you can use it by editing the caption again.
I think this is basically almost every photo that Myers has taken. What I do when I'm taking pics for a gallery is to try and get three times as many as I need, that way I can just bin the bad/blurry ones without missing out important pics. In the dark environments you have to use a pretty slow shutter and blur is inevitable since you can't always control the subject.
