Probably r_fullbright 1 with r_vertexlight 1, try to put zero on one of them.

its probably r_mapoverbrightbits and r_overbrightbits then (the combination of both is important)

maybe caused by r_lightmap 1 with r_mapoverbrightbits > 1
try setting it to 0.

If that happens every second time you start a match, try restarting your browser between each session, works for me.

my ql visuals are fucked every 2-4 time i start a match. vid_restart helps in my case, no need for restarting the browser. did you try that?

Oh yes that works too. Mine are fucked every second time. The colors differ though, I had red, green, white, black, rainbow =D. Sometimes all projectiles are neon colored, really cool, looks like a hack =)
On seldom occasions, the enemy models are bright green, but not like the /bright skins but from hair to toe, everything. Kind of nice, but I prefer the tankjr/sport skin, I got used to it.
It's fun to play like that for a while, but mostly the fonts and hud and stuff are garbled so I can't make profit out of this

rainbow quake ftw but half of the time i simply get missing or black textures for map and hud objects :| any idea what is causing it? maybe someone should report it as a bug.

It's a known bug, I found many postings about this on the quake live forum, but it's only a minor nuisance for me so I did not look into it more.
