monthly cups/leagues + monthly invitational cups/leagues for top tier players. ladders are mess.

The best thing to do would be to have ladders intergrated into the site with a leveling system, like SC2, then leagues run elsewhere to those who are top in the ladder and qualify, along with tournaments, and then invites can maybe be based on who's top in there ladder (europe, asia, north america) etc.

This is basically how EAS/EPS works over here in Europe + the top of the league (EPS) gets to play in IEM. I think a battle.net like ladder would be great if it was implemented into QuakeLive but the rest should be left out, except for maybe an invitation + travel expenses to quakecon for the top 2 of the ladder in Europe/America.

ESL is doing a great job for the most part and I think if more countries/regions pick up quake as an EPS title (or actually get an EPS) there's no need for something alike on the quakelive site. Let's see what National ESL does for America in the next few seasons.

That makes no sense whatsoever, ofcourse theres a point, it doesn't matter when it's introduced, i mean, ofcourse it's better to have it from the start, but having it would be SO much better than never having it.

It's was quite important to keep as many random people in game as possible when QL was first exposed as open beta and everyone checked it out. Now QL's audience is pretty much closed, QL is not new anymore, so no one writes news about it on popular websites, no one tells friends about it, etc, so there is no external influx of players and anything you do inside QL won't attract many people because there are almost no random people wandering on QL website as potential new players (unless Bethesda actually starts promoting QL and exposing it to external audience again).

I agree, it's a shame, but... if the game does live up to enormous standards soon, gtv, new scoreboard, ladder system then I think people will be willing to go advertise thereselves, tell friends etc. I guess we'll have to see, the game is still savable for sure imo, id just need to implement these massive changes that everyone has been asking for.

ladder would be awesome for QL, shouldn't be too hard to implement either.
