1. No irritating grapple attach noise.
2. The clicking sound when weapon selected is a little annoying, too similar to use item.
3. The hum is slightly too loud.
4. Responsive enough.
5. Can attach to NME's
6. Suggestion, when attached, you should be able to change to a weapon, so you can attach to walls / people and +attack at same time.
7. If 6 is applied, and you can change weapon while still attached, then the dmg of the Grapple is way too high.
8. Grapple dmg overall when attached is too high.

Note that grapple is now available (weapon 10).
g_velocity_gh [default 1800] is speed of grapple to target.
pmove_velocity_gh [default 800] is speed with which you are dragged to target.
Additional server settings info:
Some maps work better with PQL, some have timings enabled (start a CTF server for flags)
Maps should be run with practiceflags=1 (or practiceflags=3 for max ammo)
Freestyle maps benefits greatly from setting:
/ruleset 2
/pmove_rampjumpscale > 1
/pmove_airaccel > 1
QuakeLive Grapple is too slow. QuakeWorld was much better speed and drag. Quake Champions Athena seems to work well, but slightly over powered.

It is funny, and brings a complete new vision of maps from different angles. I love it :)
Can take a while to master well, but, it is great for noobs not being able to move as quickly as experienced players. They may still be clay pigeons sometimes, but at least they can get around, and have fun :)
And you spawn with it, "Bonus".

I am pretty sure this was on a server with syncerror though, I don't think its possible to get it on your own server. They didn't release it (yet? :D) because its too buggy when you hook other people.
Hooking other people was the fun part in QW :) I remember the first grapple in Quake 1 (b4 QW) which was the Vores pink spike homing missile as the hook part. It was classic, as you could hook an NME player just as he ran round a corner, and remain hooked, while his health went down slowly b4 death. Many funny moments trolling them while they died :)
I have just done it locally and tested the hook for the first time. I am a bit out of practise, but will uploaded a demo to youtube to show, how it spices things up a bit. I am going to try to do it on a server.. Hmm fingers crossed.
Do you still have the settings somewhere for enabling their grapple?
I don't think mine used theirs at all in the CTF. Although I thought I felt one attach to me. But it was a good fun just flying round while they were not.. :)
g_startingWeapons 512 - grapple only
bot_gauntletOnly 1 - somehow bots are using grapple more with this setting
bot_followMe 1 - optional
bot_grapple - dunno if it does anything
and give all, god etc. commands.
Although I seem to have a setting somewhere which gives me an RL and Chaingun starting with 999 ammo each, as well as gaunt + mg + hook? Weird.. mg = 100 ammo though. That is locally though.
But the "g_startingWeapons":515 works for gaunt + mg + hook, locally.
Tried to import the settings to create a server online with g_startingWeapons 515 added, but it doesn't like it ;( Unless you know how?
Yeah was hoping no one saw that one lol. I saw that after I made the demo! I have lost a year of grappling like this, that saddens me. ;)
I think from the Aerowalk part on that demo, the grapple dmg when it is attached is too high for sure.
I just played PQL CTF Grapple 1 v 4 Level 5 bots, and it was very very good fun. Unfortunately for them they didn't have grapple, nor much movement, nor starting armour, but they knew what they were doing the gits. It was still 11 caps all when I call voted cure by accident. But really good fun for any PQL and QW'ers out there.
Uploaded a PQL CTF with grapple 4 v 1 on Spider Crossings. However the QuakeLive grapple is too slow. QW was faster, Athena in Quake Champions seems to be much better.
Feels very good. (However I need to see how it plays with the bots having grapple next.)

I could imagine Weapon stay would be a popular mode.
